Ken Chance owned 4 behavioral health facilities in Arizona from 2009 – 2019, including:

• Residential Behavioral Health Facility
• Outpatient Treatment Center
• Counseling Center
• Sober Living Home

Ken wrote the policies and procedures for each facility and during one of his inspections with an ADHS surveyor, the surveyor said that she would like to see other facilities have policies and procedures laid out like Ken’s.
